2/25/2023 EnchantaVaughn Approved 1.Behavior: Anthony behaviors has been reported that he struggles with being aggressive towards peers and authority figures. Anthony indicated that he started to curse and make threats, fight and be disrespectful towards authority figures. Anthony directed to QMHP-C that client refuses to walk away for the situation and take deep breaths. Anthony also displays verbal aggression towards his siblings as evidenced by antagonizing and making threats. 2. Behavior: Anthony displayed positive and cooperative behaviors throughout the session and understands how his negative behaviors will cause him to get in trouble and his games taken away from him if he continues to present those negative behaviors. 1.Intervention: QMHP-C educated client on taking deep breaths and learn to express his emotions with a positive tone. QMHP-C educated Anthony on de-escalation techniques. QMHP-C conducted role playing activities to help client to expressing his emotions and identify the problem before reacting. QMHP-C assisted Anthony with creating a stress ball to help Anthony with controlling his anger. QMHP-C discussed anger management and positive expectation to decrease his verbal outburst. Encouraged. 2. Intervention: QMHP-C discussed Anthony mood and provided role playing activities to help Anthony with demonstrating positive facial expressions and positive communication. QMHP-C provided video that demonstrate anger outburst and utilize deep breathing, counting to ten, and walking away. QMHP-C assisted the Anthony with making a stress ball to help decrease his anger by squeezing with feeling upset. QMHP-C coached the Anthony to utilize anger management techniques to ensure Anthony making positive choices and ignoring negative situation. 1.Response: Anthony was receptive to QMHP-C's suggestions for today's session. Anthony responded with a calm and respectful tone. Anthony responded that its difficulty with expressing his anger and being aware of his emotions. Anthony was able to respond to the intervention by creating the stress ball and stated that he will try to utilize. Anthony was responsive throughout the session and understood anger management. 2. Response: Anthony responsed appropriately as evidenced by Anthony looking at the video and understanding the question provided. Anthony was appropriate by stating that he attempts to walk away and seems to get influenced by other peers. Anthony was receptive by indicating that he argues with his siblings at least3 to 4 times a week.
